Mechanizmy účinku frekventně používaných léčiv a jejich terapeutická hodnota - antiglaukomatika

Glaukom je chronická progresivní optická neuropatie, charakterizovaná exkavací papily zrakového nervu a snížením retinální citlivosti s postižením zorného pole a dalších funkcí. Jedná se o multifaktoriální onemocnění, z mnoha možných příčin poškození optického nervu jmenujme vysoký nitrooční tlak a snížený průtok krve v oku. Léčba glaukomu je zpravidla celoživotní a vyžaduje dobrou spolupráci pacienta. Uplatňují se při ní konzervativní i invazivní přístupy. Cílem léčby glaukomu je především snižování nitroočního tlaku (beta-blokátory, lokálně aplikované inhibitory karboanhydrázy, analoga prostaglandinů, kombinované preparáty), dalším přístupem je tzv. neuroprotekce, tj. snaha o zpomalení poškození gangliových buněk sítnice. V některých případech zůstává glaukom na farmakoterapii rezistentní, další možností je léčba s použitím laseru a chirurgická terapie.
